Your household income is calculated by combining the gross (pre-tax) income of all members living under the same roof. This information will be used to determine if you meet the criteria for financial assistance.

Determine Your Eligibility for Assistance

Hardship assistance is available for insured patients who cannot afford their co-payment or deductible bills. Uninsured or Non-PAR insured patients should discuss an alternative testing option with their ordering healthcare providers.

Patients may apply for a Financial Hardship Discount by completing and signing the Peak Biosciences Financial Hardship Application Form. A member if Peak Biosciences may reach out if more information is required.

FAQ

  • Why do I owe anything when I have insurance?
    Most insurance plans require you to pay a portion of your medical care as part of their coverage. The percentage you will pay varies depending on your plan. Your statement will categorize your balances as co-insurance (co-pay) or deductible, as applicable. Neither of these amounts are decided by your healthcare providers and are pre-determined by your insurance plan/insurance carrier.
  • What is the difference between a co-pay and deductible?
    Co-payment: this is the amount you will pay in addition to whatever amount your insurance has/will pay. Deductible: this is the amount you must pay out-of-pocket BEFORE your insurance will pay for your care.
  • Why don't I see any previous payments that I made on my statement?
    Our statements only list information related to current charges, balances, and insurance adjustments. If you need a payment history report, please contact us: billing@pkbiosci.com
  • What if I have no insurance or asked for my health insurance to not be billed?
    You may see a CASH fee on your statement. If you have updated billing information or want us to re-process your charges, please contact us.
  • What if I have an HMO plan?
    HMO plans require a PCP (primary care physician) referral for your medical care expenses to be covered. Without a referral, you will be required to pay out of pocket for your care. If you need assistance obtaining a PCP referral, please let us know.
  • What if I cannot afford my bill?
    We have flexible and affordable payment options, including monthly payment plans. We also offer a financial hardship discount program. More information about this plan is available below this FAQ section. Please contact us if you have questions.
  • Why did I get a bill from my insurance company?
    Any time a healthcare provider bills your insurance company, they send you remittance advice. This is not a bill. Remittance advice is informational only, please wait to receive a bill from Peak Biosciences before calling.
  • Why did my claim get denied?
    There can be several reasons for your insurance company to deny a claim, including incomplete/outdated information as well as a maximum number of tests allowed per year. Your provider will only order testing that they deem medically necessary for your care.
